What is person-centered planning?
Person-centered planning is an approach that starts with the individual’s own goals, strengths, preferences, and dreams. Instead of fitting a person into a fixed program, we design support that fits the person. The individual is the author of their own plan, with family, friends, and professionals offering guidance along the way.
How the process works
- Listen and understand. We begin by learning what matters most to the individual — their aspirations, routines, and the things that bring them joy.
- Plan together. Trained staff work with the person and their circle of support to create a tailored plan with clear, achievable goals.
- Connect and empower. We link individuals to community resources and support informed decision-making, so they have the tools and confidence to thrive.
- Review and adapt. Goals change over time, and so should the plan. We revisit it regularly to keep it meaningful.
